Surgical Observation of Bleeding Peptic Ulcers


Abstract
A surgical review was made on the cases of the bleeding peptic ulcer, treated surgically at the 16 th Army Hospital from January 1965 to December 1968.
The results were as follows:
1. The occurrence of bleeding peptic ulcers of stomach was prominent than that of duodenum, 41 cases (67.25) in stomach and 20 cases (32.8%) in duodenum.
2. Seasonal occurrence of bleeding peptic ulcers was predominant is Spring (39.3%) and Autumn (23%).
3. The duration of peptic ulcer symptoms was less than 5 years in majority (83.6%)
4. The analysis of symptoms revealed 37cases (60.7%) in hematemesis, 16 cases (26.2%) in melena and 8 cases (13.1%) in hematemesis with melena of 61 cases.
5. Of 48 cases of massive bleeding group, hypotension was in 7 cases (35.4%) and tachycardia 19 cases (39.6%).
6. Among 61 cases, 48 (78.7%) had massive bleeding and 2 cases (3.3%) expired 11 cases (18.0%) had moderate bleeding and 2 cases (3.3%) had mild bleeding.
7. Among 61 surgical procedure, subtotal gastrectomy were performed in 35 cases (57.4%), hemigastrectomy and vagotomy in 15 cases (24.6%) and pyloroplasty and vagotomy ¥± cases (18.0%).
8. The post operative complications were observed in rebleeding 6 cases (9.8%), pulmonary complication of 5 cases (8.1%), slight dyspepsia of 3 cases (4.9%) and weakness of visual acquinity of 4 cases (6.8%).
